Greenlight Laser Physics - High Power KTP Vaporisation
GreenLight PVP™ is a prostate intervention, which uses the most innovative laser technology. The GreenLight PV™ Laser System uses a Neodym:YAG laser that generates light with a wavelength of 1064 nm. It amplifies this light and bundles it. In physics, this process is called "light amplification by stimulated emission of radiation"= LASER.
Before leaving the unit, the light is conducted through a potassium titanyl phosphate = KTP crystal, whereby the laser beam doubles its length to 532 nm and is converted to a visible green light. Due to its wavelength of 532 nm (visible green light area of the colour spectrum) the high level radiation of the green light laser is particularly well absorbed by the red pigment of the red blood cells (erythrocytes) flowing through the blood vessels. That is to say, that the high-level green laser beam is blocked by the red pigments, and its high energy is immediately bundled and condensed. The green light cannot pass through the red pigment.
Within a fraction of a second upon hitting the targeted area, the light energy delivered by the laser beam is so high that the tissue instantaneously vaporises and practically dissolves. The prostate is composed of tissue filled with blood vessels. Where there are a lot of blood vessels there are also a lot of red blood cells with their typically red pigment. Thus, tissue that is richly supplied with blood is particularly well suited for green light laser therapy because, on the one hand, the penetration depth of the laser beam is limited due to the blood vessels, and on the other hand, the instantaneous vaporisation of the tissue prevents bleeding.
The Greenlight laser therapy can start. The surgeon guides a special endoscope into the urethra and pushes it to the back of the urethra. This is where the obstructive prostate tissue is located and where it constricts the urethra. Then a long, thin flexible fibre optic probe is connected to the laser unit. The other end of the laser probe protrudes from the endoscope. From the tip of the probe, the green light radiates from the side at an angle of approximately 70°. With the help of the endoscope and twisting movements, the surgeon guides the laser beam towards the obstructive prostate tissue. When the laser beam hits it mark, the top layers of tissue, which contain the blood vessels filled with red blood cells, instantly vaporise. The vaporised layer of tissue is only 1-2 mm thick. The laser cannot penetrate any further because it is blocked by the red blood pigment and the entire energy is spent during the vaporisation.
Thus, the cause of bleeding - namely the blood vessels - can no longer bleed because they were instantaneously eliminated due to the laser beam energy. Thus, step-by-step the obstructive prostate tissue can be vaporised and eliminated with virtually no bleeding. The surgeon conducts the entire procedure via a video monitor, giving him an excellent view of what is happening inside the body.
